Informed consent to virtual care
Template — pending review by legal counsel. Not yet in force.
Care delivered remotely — through questionnaires, secure messaging, and video visits — rather than an in-person examination.
Telehealth is convenient and private, but some conditions cannot be safely assessed remotely. Where that is the case, you will be referred to in-person care.
In-person care through your family physician or a walk-in clinic remains available to you at all times.
You may withdraw consent and stop using the service at any point without affecting your right to future care.
