Your information is used by your care team to assess, prescribe, and follow up on your treatment. It is not sold or used for advertising.
Access is limited to the healthcare professionals involved in your care and the accredited partner pharmacy dispensing your prescription.
Ontario law gives you the right to access your record, request corrections, and ask how your information has been used or disclosed.
Records are stored in Canada on a PHIPA-aligned platform and retained for the period required of medical records in Ontario.
Questions about your information can be directed to the clinic through your patient portal.
